一、經學校查核有部分同學(含碩、博士班)於辦理入學註冊，進行線上網路個人綜合資料表登錄時，並未詳實填寫，後續亦未至生輔組繳交個人兵役基本資料表，就算已繳交，亦未將個人身分證正反面影本及免服役或已服常備役(含替代役)之退伍證明文件附上，以致造成作業比對困難，無法確認個人所載資料是否正確，因而延宕學校向各級縣市政府及地區後備指揮部陳報個人有關兵役緩徵及後備軍人儘後召集申請作業之時效性，若已繳交相關佐證文件者，可不予理會。
二、煩請同學們務必再三自我檢視，如有前揭陳述情形者，因攸關個人權益，請務必於09月30日前速將兵役基本資料表(如附件一)繳交至中央校區、介仁校區與建國校區生輔組(中央校區-程曾廣智校安、介仁校區-張世超校安、建國校區-王曉涵)，若因未繳交或遲交，而導致衍生後續有關兵役方面之問題者，後果自負。
三、若欲辦理延長修業兵役緩徵者，亦請填寫延畢生身分確認申請表(如附件二)，並經相關師長審核簽章後，繳交至前揭所述地點。

I. Upon review by the university, it has been found that some students (including those in master's and doctoral programs) did not provide accurate information when completing the online Personal Comprehensive Information Form during the enrollment registration process. Furthermore, they did not submit the Personal Military Service Basic Information Form to the Student Affairs Office. Even for those who have submitted the form, some failed to attach copies of both sides of their national identification card and proof of exemption from military service or proof of completion of regular military service (including alternative service). This has caused difficulties in verifying the accuracy of the information provided, thereby delaying the university's timely submission of applications for deferment of military service and subsequent recall of reserve personnel to the relevant county and city governments and regional reserve command headquarters. Students who have already submitted the required supporting documents may disregard this notice.
II. Please kindly ensure that all students thoroughly review their information. If you fall under the aforementioned circumstances, please submit the Personal Military Service Basic Information Form (see Attachment 1) to the Student Affairs Office at the Central Campus, Jie-Ren Campus, and Jian-Guo Campus (Central Campus - Cheng Tseng-Guang, Jie-Ren Campus - Chang Shih-Chao, Jian-Guo Campus - Wang Hsiao-Han) by September 30. Any subsequent issues related to military service arising from failure to submit or late submission of the required documents will be the responsibility of the student.
III. For students wishing to apply for an extension of their period of study to defer military service, please also complete the Application Form for Confirmation of Identity of Students Requesting an Extension (see Attachment 2), obtain the necessary review and signature from the relevant faculty members, and submit it to the aforementioned locations. |
