為利本校向教育部提報117學年度大學校院申請增設、調整【特殊項目】院系所學位學程案，貴單位若需申請下列情事之一者：
一、博士班增設(含既有博士班擬轉型為跨院、系、所性質之學院博士班或博士學位學程)或涉及領域變更之調整案。
二、醫事相關類科之院、系、所、學位學程增設、及涉領域變更之調整案，應於本次特殊項目申請時程提報，屬醫事相關類科案件分述如下：
１、屬碩士班增設、調整案：臨床心理師（臨床心理、行為醫學相關研究所）、諮商心理師（諮商心理相關研究所）、獸醫(校內未設有獸醫學系，但欲申設獸醫研究所、獸醫碩士學位學程）之碩士班案。
２、屬學士班增設、調整案：醫師（醫學系）、中醫師（中醫學系）、牙醫師（牙醫學系）、護理師（護理學系）、藥師（藥學系）、醫事檢驗師（醫事檢驗相關學系）、物理治療師（物理治療、復健學系）、職能治療師（職能治療、復健學系）、醫事放射師（醫事放射相關學系）、牙體技術師（牙體技術學系）、獸醫（獸醫、畜牧獸醫學系）、驗光師(視光學系)。
３、屬含碩士班或學士班增設、調整案：助產師（助產學系、所）、營養師（營養相關學系、所）、呼吸治療師（呼吸治療、呼吸照護學系、所）、語言治療師（語言治療、溝通障礙學系、所）、聽力師（聽力、溝通障礙學系、所）。
三、師資培育相關學系增設、調整案。
◎增設提案數：
１、【特殊項目】及【一般項目】各學制班別增設案合計至多各3案。
２、若超過3案需於校務發展委員會議或校務會議審議排序，第4案以後之案件(教育部)將不予受理審查。
請於115年9月23日前將「117學年度申請增設、調整【特殊項目】及【一般項目】院、系、所、學位學程計畫書」紙本及電子檔，經系/所/學位學程會議、院務會議審核後送教務處招生策略中心，以利逕送校務發展委員會會議、(1151007)校務會議及董事會議審議。

To facilitate the submission of applications for the establishment and adjustment of academic programs under the "Special Items" category for the 117th academic year to the Ministry of Education, units within this university wishing to apply for any of the following matters are requested to proceed as follows:
1. Applications for the establishment of doctoral programs (including the transformation of existing doctoral programs into cross-college, department, or institute doctoral programs or doctoral degree programs) or adjustments involving changes in academic fields.
2. Applications for the establishment and adjustment of academic programs related to medical and health professions under the "Special Items" category must be submitted within the specified timeline for this application. These cases are detailed as follows:
1. Applications for the establishment and adjustment of master's programs: Clinical Psychologist (Clinical Psychology and Behavioral Medicine-related Graduate Programs), Counselor Psychologist (Counseling Psychology-related Graduate Programs), Veterinarian (Veterinary Science-related Graduate Programs or Master's Degree Programs in Veterinary Science, where the university does not currently have a Veterinary Science Department).
2. Applications for the establishment and adjustment of bachelor's programs: Physician (Department of Medicine), Traditional Chinese Medicine Physician (Department of Traditional Chinese Medicine), Dentist (Department of Dentistry), Nurse (Department of Nursing), Pharmacist (Department of Pharmacy), Medical Laboratory Scientist (Medical Laboratory Science-related Departments), Physical Therapist (Departments of Physical Therapy and Rehabilitation), Occupational Therapist (Departments of Occupational Therapy and Rehabilitation), Radiologic Technologist (Radiologic Technology-related Departments), Dental Technologist (Department of Dental Technology), Veterinarian (Departments of Veterinary Science and Animal Husbandry and Veterinary Science), Optometrist (Department of Optometry).
3. Applications for the establishment and adjustment of master's or bachelor's programs: Midwife (Departments or Institutes of Midwifery), Dietitian (Nutrition-related Departments or Institutes), Respiratory Therapist (Departments or Institutes of Respiratory Therapy and Respiratory Care), Speech Therapist (Departments or Institutes of Speech Therapy and Communication Disorders), Audiologist (Departments or Institutes of Audiology and Communication Disorders).
3. Applications for the establishment and adjustment of academic programs related to teacher education.
**Number of Establishment Proposals:**
1. The total number of proposals for the establishment of programs under both the "Special Items" and "General Items" categories is limited to three per academic level.
2. If the number of proposals exceeds three, they must be reviewed and prioritized by the University Development Committee or the University Council. Proposals ranked fourth or lower will not be accepted for review by the Ministry of Education.
Please submit the paper and electronic versions of the "117th Academic Year Application for the Establishment and Adjustment of [Special Items] and [General Items] Academic Programs" by September 23, 115. These documents must be reviewed and approved by the department/institute/degree program meeting and the college meeting before being sent to the Admissions Strategy Center of the Academic Affairs Office for further submission to the University Development Committee, the (1151007) University Council, and the Board of Directors for review.
