一、敬請各學院協助辦理『114學年度優良導師』候選人推薦事宜，並於115年9月30日(三)前檢具以下資料，向本校「優良導師評選委員會」推薦(承辦單位：學生諮商中心)：
(一)遴選會議紀錄
(二)優良導師推薦表
(三)被推薦導師最近一年輔導工作佐證資料
二、收件採電子檔方式，雲端上傳連結將另行提供予院助理。紙本或需用印資料請掃描後上傳。請留意學生隱私，資料匿名或去識別化處理。
三、依據優良導師獎勵辦法第四條規定推薦程序如下：
1.由各院系所依據第二條第二款獎勵事項與績效事實，經系所(含師資培育中心)會議討論議定，提供優良導師推薦表(如附件)及會議紀錄影本，向各學院進行推薦。各系所推薦至學院之人選以至少一位為原則。
2.各學院需於評選公告期限前，就各系所推薦名單召集會議，審查推薦人選，並檢附優良導師推薦表、最近一年輔導工作佐證資料及會議紀錄，向校級優良導師評選委員會推薦。
四、經115年6月15日召開之優良導師評選籌備會議決議，訂定114學年度各學院可推薦之優良導師候選人名額如下：
醫學院10位(臨床醫師導師保障推選名額1名)、護理學院10位(五專部導師保障推選名額3名)、工程科技學院2位、人文傳播暨社會科學院4位、智慧永續管理學院3位，總計29位。
五、優良導師評選會議預定於115年10月底辦理，候選導師請準備「4分鐘演說簡報」闡述其輔導理念、具體作為及成效。如有正當事由無法出席者，得以錄影為之。
六、敬附優良導師獎勵辦法及相關表件，以供參考。

**Announcement Regarding the Nomination of Excellent Mentors for the 114th Academic Year**
The Office of Student Counseling kindly requests the assistance of all colleges in nominating candidates for the "Excellent Mentor" award for the 114th academic year. Please submit the following documents to the "Excellent Mentor Selection Committee" by September 30, 2026 (Monday):
1. Minutes of the selection meeting
2. Nomination form for Excellent Mentor
3. Supporting documents for the nominee's mentoring work in the past year
Submissions should be made electronically via a cloud upload link that will be provided to each college's administrative assistant. If a paper copy or a document requiring an official seal is necessary, please scan it and upload the file. Please ensure that student privacy is protected by anonymizing or de-identifying the data.
According to Article 4 of the Reward Measures for Excellent Mentors, the nomination procedure is as follows:
1. Each department or institute should nominate candidates based on the criteria and performance outlined in Article 2, Section 2 of the Reward Measures. After discussion and approval at the departmental meeting (including the Center for Teacher Education), submit the nomination form (see attachment) and a copy of the meeting minutes to the respective college. Each department should nominate at least one candidate.
2. Each college must convene a meeting before the deadline for nominations to review the recommended candidates from its departments. The college should then submit the nomination form, supporting documents for the nominee's mentoring work in the past year, and the minutes of the review meeting to the University-level Excellent Mentor Selection Committee.
Based on the resolution of the preparatory meeting for the Excellent Mentor Selection held on June 15, 2026, the number of candidates that each college may nominate for the 114th academic year is as follows:
- College of Medicine: 10 candidates (with 1 guaranteed nomination for a clinical mentor)
- College of Nursing: 10 candidates (with 3 guaranteed nominations for mentors in the Five-Year Nursing Program)
- College of Engineering and Technology: 2 candidates
- College of Humanities, Communication, and Social Sciences: 4 candidates
- College of Smart and Sustainable Management: 3 candidates
Total: 29 candidates
The Excellent Mentor Selection Meeting is scheduled to take place at the end of October 2026. Nominees are requested to prepare a 4-minute presentation to elaborate on their mentoring philosophy, specific actions taken, and the outcomes achieved. If a nominee is unable to attend due to valid reasons, a video submission may be made instead.
The Reward Measures for Excellent Mentors and related documents are attached for your reference.
