一、本院為提升臨床照護品質，以產學合作共同培育學生並提供就業機會，以提升市民照護品質，同時關懷照顧經濟弱勢及原住民學生，鼓勵長期照顧/老人/高齡/銀髮/長青相關學科系，且該學門學程臨床實習以在本院執行為優先之優秀學生畢業後從事臨床照顧服務，提供長期照顧相關科系學生獎助金申請，敬請貴校惠予公告並鼓勵推薦符合資格學生踴躍申請。
二、獎助對象：
(一)全修業期限皆能申請(高中職三年、五專五年、大學四年)之長期照顧相關科系學生。
(二)之前申請本院一學期以上之獎助金學生（名單如附件）。
三、獎助條件：受獎助學生在申請前一學期之學業成績平均75分以上，且操行成績80分以上。獎助內容及期間：依申請任職年限獎助，每學期獎助金新臺幣(以下同)六萬元整。受獎助學生須簽定「臺北市立聯合醫院提供院校長期照顧相關科系學生獎助金服務合約書」，畢業後依據本院規定到職日履行保證義務年限，從事病房助理員或照顧服務員職務。(例如:申請二學期獎助金，其服務年限為服務一年)。
四、申請方式:符合獎助對象之學生，填妥「臺北市立聯合醫院提供院校長期照顧相關科系學生獎助金申請表」，並依申請表檢附資料查檢表依序檢附相關資料，經學系(科)用印後於115年09月30日(三)前送至本院護理部審查(10341台北市大同區鄭州路145號院本部4樓護理部)，逾期不再受理申請(郵戳為憑)。
五、一年級上學期申請者先提送申請，後補資料核發經費方式如下說明：一上學期結束後，補送成績單，確認符合補助者再核發費用。
六、第一次申請之學生，請提供台北富邦銀行或第一銀行的帳戶(帳戶戶名須為學生本人)。
七、請務必使用當年度公告之申請表格申請，合約書及領款收據有塗改處，請務必蓋章或簽名。

I. To enhance the quality of clinical care, this hospital has established industry-academia partnerships to jointly train students and provide employment opportunities, thereby improving the quality of care for citizens. At the same time, to support economically disadvantaged and Indigenous students, the hospital encourages outstanding students from long-term care/ gerontology/elderly care/silver-haired/senior care-related disciplines—with priority given to those whose clinical internships are conducted at this hospital—to pursue careers in clinical care services upon graduation. We are therefore offering scholarship and grant opportunities to students in long-term care-related programs. We kindly request that your institution publicize this announcement and encourage eligible students to apply actively.
II. Eligibility:
(1) Students enrolled in long-term care-related programs who are eligible to apply throughout their entire course of study (three years for senior high school and vocational high school; five years for five-year technical colleges; four years for universities).
(2) Students who have previously received grants from our hospital for one semester or more (list attached).
III. Eligibility Criteria: Recipients must have achieved an academic GPA of 75 or higher and a conduct grade of 80 or higher in the semester prior to application. Award Details and Duration: The award is based on the required length of service after graduation, with a stipend of NT$60,000 (New Taiwan Dollars; hereinafter the same) per semester. Recipients must sign the “Taipei City United Hospital Service Contract for Grants Provided to Students in Long-Term Care-Related Programs at Educational Institutions” and, upon graduation, fulfill the required service period as stipulated by the Hospital, working as a ward assistant or care attendant. (For example: If a student applies for two semesters of grants, the required service period is one year.)
IV. Application Procedure: Eligible students must complete the “Taipei Municipal United Hospital Scholarship Application Form for Students in Long-Term Care-Related Programs at Institutions of Higher Education” and attach the required supporting documents in the order specified in the Application Form Document Checklist. After obtaining the official seal from their department (or program), the application must be submitted to the Hospital is Nursing Department for review by Wednesday, September 30, 115. (Nursing Department, 4th Floor, Main Campus, No. 145 Zhengzhou Road, Datong District, Taipei City 10341). Applications received after the deadline will not be accepted (as evidenced by the postmark).
V. For first-year students applying during the first semester, the application must be submitted first, and the process for supplementing documents and disbursing funds is as follows: After the first semester ends, submit your transcript; funds will be disbursed only after eligibility is confirmed.
VI. Students applying for the first time must provide a bank account with Taipei Fubon Bank or First Bank (the account holder is name must be the student is own).
VII. Please be sure to use the application form announced for the current academic year. If there are any alterations on the contract or payment receipt, please be sure to affix a seal or sign your name.
