一、依據本校學生修讀輔系辧法、本校學生加修雙主修辦法、臺灣綜合大學系統辦理跨校輔系辦法、臺灣綜合大學系統辦理跨校雙主修辦法、本校與高雄醫學大學辦理跨校輔系辦法、本校與高雄醫學大學辦理跨校雙主修辦法及簽有校級學術合作協議之學校規定辦理。
二、申請期間：115年8月3日至8月14日中午12時截止。
三、申請程序：(一)擬申辦之學生，即日起可至本校教務處網頁下載申請表(路徑：學生專區-學籍及成績-輔系/雙主修)。(二)填妥申請表後請連同歷年成績單正本，至原校所屬系所及教務處辦理審查。未於申請期間內送件者，逾期不予受理。(三)請原校教務處彙送學生申請資料，並於8月14日下午5時3第1頁，共2頁前email至本校教務處註冊組(hsiuwen@mail.nsysu.edu.tw）進行審查。(四)申請流程、申請表請參閱附件一、二。

**Announcement Regarding Application for Minor and Double Major Programs**
**I. Basis for Implementation**
This announcement is made in accordance with the following regulations: the Rules for Students Pursuing a Minor at this University, the Rules for Students Pursuing a Double Major at this University, the Taiwan Consortium of Universities i Rules for Inter-university Minor Programs, the Taiwan Consortium of Universities i Rules for Inter-university Double Major Programs, the Rules for Inter-university Minor Programs between this University and Kaohsiung Medical University, the Rules for Inter-university Double Major Programs between this University and Kaohsiung Medical University, and the regulations of universities with which this University has signed inter-university academic cooperation agreements.
**II. Application Period**
The application period is from August 3, 2026, to August 14, 2026, with the deadline at 12:00 noon on August 14, 2026.
**III. Application Procedure**
(1) Students wishing to apply may download the application form from the Office of Academic Affairs i website starting today. The path to the form is: Student Zone - Academic Records - Minor/Double Major.
(2) After completing the application form, please submit it along with the original copies of your academic transcripts to your home department and the Office of Academic Affairs for review. Applications not submitted within the specified period will not be accepted.
(3) The Office of Academic Affairs at the home university is requested to compile and email the students i application materials to the Registration Section of the Office of Academic Affairs at this University (hsiuwen@mail.nsysu.edu.tw) by 5:30 PM on August 14, 2026, for review.
