各位教職同仁您好:
一、依據本校《慈濟大學關懷互助基金管理辦法》第四條第一款規定:「除新進人員得以到職日起選擇加入外,之後均以學年度為期申請加入或退出,且退會者不另予退回已扣繳之費用。」
二、請有意申請加入或退出關懷互助會之同仁,於115年7月17日(星期五)下班前填妥「加/退意願書」(附件一),經親筆簽名或蓋章後,送交 人資處潘筱雯同仁 辦理。
三、申請加入者,請一併填寫全球人壽要保書(附件二)。
四、申請之生效日期為115年8月1日;申請退出者,已扣繳之費用恕不退還。
五、請務必於期限內完成申請,逾期恕不受理;學年度進行期間亦不受理加入或退出申請。
六、有關本基金之互助事項及相關辦法,請參閱附件三。

Dear Faculty and Staff Members,
1. According to Article 4, Paragraph 1 of the "Tzu Chi University Care and Mutual Assistance Fund Management Regulations," new employees may choose to join the Fund upon their date of employment. Subsequently, applications to join or withdraw from the Fund must be submitted on an academic year basis. Withdrawals will not be eligible for the refund of any previously deducted fees.
2. Faculty and staff members wishing to apply for membership or withdrawal from the Care and Mutual Assistance Fund are requested to complete the "Membership/Withdrawal Intention Form" (Attachment 1), sign it in person, and submit it to Ms. Pan Hsiao-Wen at the Human Resources Office by 17th July, 2026.
3. Those applying to join the Fund are also required to complete the insurance application form (Attachment 2).
4. The effective date for all applications will be 1st August, 2026. Please note that any fees already deducted for withdrawal applications will not be refunded.
5. All applications must be submitted within the specified deadline; late submissions will not be accepted. Applications to join or withdraw will not be processed during the academic year.
