各位同仁您好：
1.依據本校 關懷互助基金管理辦法第五條 辦理身心健康補助：
(一) 同仁參加本校社會教育推廣處舉辦之課程，每學年補助二次，一次300元，一學年上限600元。
(二) 同仁辦理大喜館學期使用證，每學期補助360元，繳費兩期(一年)，則視同兩次，得給予補助720元。
2. 參加 社會教育推廣處 課程，每學年最高補助二次(即二門課程)，若上學期已申請二門課程者，請勿重覆申請。
3.非本學年度案件，不予受理。
敬請會員於即日起至115/06/30止，將「補助申請單(如附件，請勿使用舊版表單)」及「繳費收據影本」送至承辦人：中央校區事務組潘杰秀小姐(#11320)，以辦理申請補助，逾期恕不受理。

Dear Colleagues,
In accordance with Article 5 of the University is Caring and Mutual Assistance Fund Management Regulations, physical and mental health subsidies are provided as follows:
(1) Colleagues who participate in courses organized by the University is Office of Continuing Education are eligible for a subsidy of NT$300 per course, up to two times per academic year, with a maximum annual subsidy of NT$600.
(2) Colleagues who apply for a semester pass for the Daxi Hall are eligible for a subsidy of NT$360 per semester. Payment for two semesters (one academic year) will be counted as two applications, with a total subsidy of up to NT$720.
2. For courses offered by the Office of Continuing Education, the maximum subsidy is limited to two courses per academic year. If you have already applied for subsidies for two courses in the first semester, please do not apply again.
3. Applications for cases outside the current academic year will not be accepted.
Members are requested to submit a copy of the payment receipt and the subsidy application form (see attached; please do not use old versions of the form) from now until June 30, 2026. Late applications will not be accepted.
