【畢業班導師請注意】1142學期學生操行評分
一、本學期畢業班操行評分將於115年5月18日(一)開放至115年5月25日(一)下午17點關閉。
非畢業班預計在115年6月8日開放至115年6月15日(一)下午17點關閉，此部分會另外公告。
二、請各位畢業班師長至校務資訊系統>學務處>生活輔導組>新增操行成績，相關作業介面請閱附件。
有設導師之班級>>請各位導師進行評分
未設導師之班級>>請系(所)主任進行評分
三、依本校學生操行成績考核辦法，操行成績計算評定公式如下：操行成績=基本分82分＋組導師共加減1~4分＋生活輔導組教官(校安)加減1~4分＋缺曠課及獎懲加扣分（導師評分學生表現，加減滿4分須於備註說明原因）。
四、感恩老師協助，有任何疑問，請洽學務處生活輔導組-分機11203。
備註：60分以下操行不及格者，預計於期末學生獎懲委員會提案。

【Graduation Class Advisors, Please Take Note】Student Conduct Evaluation for the 1142 Semester
1. The conduct evaluation for graduation class students will be open from Monday, May 18, 2026, to Monday, May 25, 2026, at 17:00. For non-graduation classes, the evaluation will be open from Monday, June 8, 2026, to Monday, June 15, 2026, at 17:00. Further announcements will be made regarding this part.
2. Please proceed to the Academic Affairs Information System > Student Affairs Office > Life Guidance Section > Add Conduct Score for the evaluation. For classes with assigned advisors, please conduct the evaluation. For classes without assigned advisors, please have the department (institute) head perform the evaluation. Please refer to the attached document for the relevant interface.
3. According to the Student Conduct Score Evaluation Regulations of this university, the calculation formula for conduct scores is as follows: Conduct Score = Base Score of 82 + Group Advisor Adjustment of ±1 to ±4 + Life Guidance Section Instructor (Campus Safety) Adjustment of ±1 to ±4 + Adjustments for Absences and Awards/Punishments (Advisors must provide reasons in the remarks if adjustments reach ±4).

Note: Students with a conduct score below 60 will be proposed to the End-of-Term Awards and Punishments Committee.
