一、依據「慈濟大學輔導學分學程修習辦法」第五條辦理。
二、請欲申請「輔導學分學程證明書」同學，請於5月18日(星期一)前將「學程證明申請書」、歷年成績單一份及當學期選課確認單，送至師資培育中心(介仁校區明心樓2樓2E216辦公室，03-8572677分機33166)。
1.This announcement is issued in accordance with Article 5 of the Regulations for the Study of the Counseling Credit Program of Tzu Chi University.
2.Students wishing to apply for the Certificate of the Counseling Credit Program are requested to submit the following documents to the Center for Teacher Education (Room 2E216, Mingxin Building, 2nd Floor, Miaoren Campus, Tzu Chi University, Extension 33166, Tel: 03-8572677) by Monday, May 18:
- Application Form for the Program Certificate
- One copy of the academic transcript
- Course selection confirmation for the current semester |
