※申請資格：參閱附件。

※申請文件請參照申請辦法所要求之提供，請勿提供沒要求之其餘證明，節省紙張用量，暨環保節又節能減碳。
※如果您不符合以上資格，請勿強行送件，不符資格者一律退件。
