一、 重要提醒:
1. 本項獎助金係依據《慈濟大學清寒助學金辦法》辦理。
2. 請同學務必提醒導師上網填寫導師意見表,做為審查依據。
二、申請期間:1150622-1150625止(學生資訊系統將於此期間開放申請,並須同步繳交紙本資料)
三、資料繳交地點與窗口:各校區學務處可代收。

四、申請資格:
1.學業(智育)成績須達 65 分(含)以上,德育(操行)成績須達 80 分(含)以上,且無小過(含三支申誡累計)以上處分紀錄(含銷過前紀錄)。
2.前一學期完成志工服務及人文時數合計至少10小時(不得重複計入申請本校其他獎助學金所要求之時數),並須於第18週(含)前完成登錄。
五、應繳資料:(繳交前請詳閱並確認資料正確)
1.學生系統申請書(含同意書)—學生本人務必簽名。
2.戶籍謄本正本(戶籍若不同請分別附上) 。
※115-1開學日為9月,戶籍謄本(需三個月內)正本需115年6月以後開立之,或新式戶口名簿完整影本(含父母/監護人與學生本人,含記事欄),若繳交戶口名簿影本,須另附內政部「戶口名簿請領紀錄證明」。
3. 三個月內開立之下列任一佐證資料(擇一提供,須含學生姓名):
※清寒證明正本。(證明需為115年5月之後開立)
※低收入戶證明正本。(證明需為115年)
※中低收入戶證明正本。(證明需為115年)
4. 財政部國稅局核發之113年度所得證明(需含父母/監護人與學生本人)。
**Announcement: Application for Tzu Chi University Merit-based Scholarship for Students in Financial Need**

**I. Important Reminders:**
1. This scholarship is administered in accordance with the "Tzu Chi University Merit-based Scholarship for Students in Financial Need Regulations."
2. Please remind your academic advisor to complete the online advisor recommendation form, which will serve as a basis for the review process.
**II. Application Period:**
From June 22, 2026 (1150622) to June 25, 2026 (1150625). The Student Information System will be open for online applications during this period, and paper documents must be submitted simultaneously.
**III. Submission Location and Contact:**
Applications may be submitted at the Student Affairs Office of each campus.

**IV. Eligibility Criteria:**
1. Academic (Intellectual) performance must be 65 points or higher, and Moral (Character) performance must be 80 points or higher. Applicants must not have received any minor or more severe disciplinary actions (including cumulative warnings), including records prior to any expungement.
2. Applicants must have completed a combined total of at least 10 hours of volunteer service and humanities activities in the previous semester. These hours may not overlap with those required for other scholarship applications. All hours must be logged by the 18th week of the semester (inclusive).
**V. Required Documents:**
(Please review and confirm the accuracy of all documents before submission.)
1. Application Form from the Student System (including consent form)—must be personally signed by the student.
2. Original household registration extract (if the student is household registration differs from that of their parents/guardians, please submit separate extracts).
*For the academic year 115-1 (commencing in September), the household registration extract must be issued after June 2026 (within three months). Alternatively, a complete copy of the new-style household register (including the student, parents/guardians, and the annotation section) may be submitted. If the household register is provided, please also submit a copy of the "Household Register Issuance Record Certificate" issued by the Ministry of the Interior.*
3. Any one of the following supporting documents, issued within the last three months and containing the student is name (choose one):
*Original certificate of financial hardship (issued after May 2026).*
*Original certificate of low-income household (issued in 2026).*
*Original certificate of medium-low-income household (issued in 2026).*
4. Income certificate for the year 2024, issued by the National Taxation Bureau (must include the student, parents, or guardians).
