為便利每學年彙整提供優良導師業務單位，自114-02學期起，請師長於關懷輔導學生傷病後上校務系統填寫學生傷病訪視輔導表(填寫說明如附件)，並列 印併同發票(收據)送學務處辦理結報事宜。
To facilitate the annual consolidation and reporting of outstanding faculty advisors, starting from the 114-02 semester, please ensure that all faculty members complete the Student Illness/Injury Visit and Counseling Form on the university administrative system after providing care and guidance to students who have experienced illness or injury. Please refer to the attached instructions for form completion. After submission, kindly print the form and submit it together with the invoice (receipt) to the Student Affairs Office for reimbursement processing. |
