說明：
一、本院為提升臨床醫護水準，以產學合作共同培育學生並提供適當的就業機會，提升市民醫療品質，同時關懷照顧經濟弱勢及原住民學生，鼓勵護理科系優秀學生畢業後從事護理臨床照顧服務，提供護理相關科系學生獎助金申請。
二、獎助對象：
(一)大專院校護理科系或護理助產科系學生。
(二)之前申請本院一學期以上之獎助金學生（名單如附件）。
三、獎助條件：
(一)受獎助學生在申請前一學期之學業成績平均75分以上，且操行成績平均80分以上，符合資格者依名額擇優獎助。
(二)另與本院簽屬此案產學合作合約書之大專院校所推薦之獎助學生，亦列為擇優獎助對象，且校方未來與本院申請之護理實習梯次同樣優先篩選。
四、獎助內容及期間：依申請任職年限獎助，每學期獎助金新臺幣(以下同)六萬元整。
五、受獎助學生需簽定「臺北市立聯合醫院提供院校護理相關科系學生獎助金服務合約書」，畢業後依據本院規定到職日履行保證義務年限。(例如：申請二學期獎助金，其服務年限為服務一年。)
六、申請方法：符合獎助對象之學生，填妥「臺北市立聯合醫院提供院校護理相關科系學生獎助金申請表」，而第2次(含)以上領取獎助金學生，填妥「臺北市立聯合醫院提供院校護理科系學生獎助金申請表-第2次(含)以上領取者」，並依申請表檢附資料查檢表依序檢附相關資料，經學系（科）用印後於115年03月15日（日）前送至本院護理部審查，逾期不再受理申請(郵戳為憑)。
七、一年級上學期申請者先提送申請，後補資料核發經費方式說明如下：一上學期結束後，補成績單，確認符合補助者再核發費用。
八、第一次申請之學生，請提供台北富邦銀行或第一銀行的帳戶 (帳戶戶名須為學生本人)。
九、檢附獎助金申請須知、獎助金申請表、獎助金服務合約書各一份提供參考。
十、請務必使用當年度公告之申請表格申請，合約書及領款收據有塗改處，請務必蓋章或簽名。
十一、申請資料郵寄地址：10341臺北市大同區鄭州路145號院本部4F護理部收。

**Announcement: Application for Nursing Scholarship Provided by Taipei City Hospital System**

**Purpose:**
To enhance clinical nursing standards, the Taipei City Hospital System collaborates with academic institutions to jointly cultivate students and provide appropriate employment opportunities. This initiative aims to improve the quality of healthcare services for citizens while supporting economically disadvantaged and indigenous students. We encourage outstanding graduates from nursing-related disciplines to pursue careers in clinical nursing care by offering scholarship opportunities.

**Eligibility:**
1. Students enrolled in undergraduate or associate degree programs in Nursing or Nursing Midwifery at accredited institutions.
2. Students who have previously received this scholarship for at least one semester (see attached list).

**Eligibility Criteria:**
1. Applicants must have an academic average of 75 or above and a conduct average of 80 or above in the semester prior to application. Qualified candidates will be selected based on merit within the allocated quota.
2. Students recommended by institutions that have signed a cooperative agreement with the Taipei City Hospital System are also eligible for consideration. These institutions will also receive priority in future nursing internship placements.

**Scholarship Details and Duration:**
The scholarship amount is NT$60,000 per semester, contingent upon the duration of service commitment as specified in the application.

**Obligations of Recipients:**
Recipients must sign a Service Agreement with the Taipei City Hospital System. Upon graduation, they are required to fulfill their service obligation according to the terms outlined in the agreement (e.g., a two-semester scholarship requires one year of service).

**Application Process:**
1. Eligible students must complete the "Application Form for Nursing Scholarship Provided by Taipei City Hospital System."
- For students applying for the scholarship for the second time or more, please use the "Application Form for Nursing Scholarship Provided by Taipei City Hospital System—Second Application and Beyond."
2. Attach all required documents as listed on the checklist and submit them to the Nursing Department of the Taipei City Hospital System by March 15, 2024 (Sunday). Late submissions will not be accepted (postmark date applies).

**Special Notes for First-Semester Applicants:**
Applicants who submit their application during the first semester must provide their final transcript after the semester concludes. Funds will be disbursed only after verification of eligibility.

**Bank Account Requirement:**
First-time applicants must provide a bank account from either Fubon Bank or First Bank (account holder must be the student themselves).
