一、依據衛生福利部國民健康署115年度補助地方辦理「整合性長者健康促進計畫-工作項目1長者健康促進服務」及「長者健康促進服務方案與師資管理指引」辦理。
二、辦法摘要如下：
(一)申請期間：自即日起至115年9月30日止。
(二)申請資格：具本縣合格之長者健康促進服務方案研發單位。
(三)申請次數：115年每方案至多申請2次(包含年度查核輔導訪查依委員意見所進行之方案模組異動)。
(四)方案變更異動範圍：詳變更申請辦法。
三、如有方案模組異動需求，請於申請截止日前檢附方案異動申請書、異動後之「長者健康促進服務方案模組提報表」及其他相關佐證資料1份併同電子檔光碟函提供給研發處智慧財產及產學創業中心。

**Announcement Regarding the Application for the "Integrated Elderly Health Promotion Program" under the National Health Insurance Administration, Ministry of Health and Welfare**
1. This announcement is issued in accordance with the guidelines for the 2026 fiscal year grant provided by the National Health Insurance Administration, Ministry of Health and Welfare, for local implementation of the "Integrated Elderly Health Promotion Program—Work Item 1: Elderly Health Promotion Services" and the "Guidelines for Elderly Health Promotion Service Programs and Faculty Management."
2. Summary of the Application Process:
(1) **Application Period:** From the date of this announcement until September 30, 2026.
(2) **Eligibility:** Qualified organizations within this county that have developed Elderly Health Promotion Service Programs.
(3) **Application Frequency:** Each program may be submitted for review a maximum of two times in 2026 (including any changes to program modules based on feedback from annual audits and advisory visits).
(4) **Scope of Program Changes:** Refer to the guidelines for application procedures regarding changes.
3. If there is a need to modify any program modules, applicants must submit the following documents to the Intellectual Property and Industry-Academia Innovation Center of the Research and Development Office before the application deadline:
- Application form for program changes
- Updated "Elderly Health Promotion Service Program Module Submission Form"
- Other relevant supporting documents (one hard copy and one electronic copy on CD)
