一、依據衛生福利部國民健康署115年度補助地方政府辦理「整合性長者健康促進計畫-工作項目1長者健康促進服務」及「長者健康促進服務方案與師資管理指引」辦理。
二、計畫摘要如下：
(一)申請單位：具本縣合格之長者健康促進服務方案研發單位。
(二)計畫目的：
1、鼓勵相關人力投入長者健康促進方案教學市場並提升民眾健康意識及自主健康管理，厚植本縣方案師資庫。
2、鼓勵本縣方案單位至外縣市辦理新師資培訓，推廣本縣方案。
三、執行期間：自核定日起至115年9月30日止
四、請依申請須知踴躍研提計畫，並於截止日前將申請計畫書、合作意向書(如涉及原方案模組異動，請另檢附方案異動申請書、異動後之長者健康促進服務方案模組提報表)各1式7份併同電子檔光碟1份供給研發處智慧財產及產學創業中心。

1. Authority and Basis This program is implemented in accordance with the Health Promotion Administration (Ministry of Health and Welfare) 2026 subsidies for local governments: "Integrated Elderly Health Promotion Plan – Work Item 1: Elderly Health Promotion Services" and the "Guidelines for Elderly Health Promotion Service Schemes and Instructor Management."
2. Program Summary
Eligible Applicants: Certified research and development (R&D) units of elderly health promotion service schemes within Hualien County.
Program Objectives:
To encourage human resources to enter the elderly health promotion teaching market, enhance public health awareness and self-health management, and expand the county is instructor talent pool.
To encourage local scheme units to conduct new instructor training in other counties to promote Hualien is service schemes.
3. Execution Period From the date of approval until September 30, 2026.
4. Submission Instructions Units are encouraged to propose programs according to the application guidelines. Please submit 7 copies each of the "Program Proposal" and "Letter of Intent for Cooperation" (if any modifications to the original scheme modules are involved, please also include the "Scheme Modification Application Form" and the "Revised Elderly Health Promotion Service Scheme Module Report Form"), along with 1 electronic copy on CD, to the Intellectual Property and Center for Academia-Industry Collaboration of the Office of Research and Development before the deadline.
