各位師長、同仁，大家好：
為維護教職員工健康，115年度定期健康檢查預計於三至四月期間辦理。為確保慈濟醫院之醫療量能與健檢服務品質，並精確配置各校區之醫護人力，學校須事先統計各項健檢之受檢人數與意願。
本次將透過 E-mail 個別寄送個人專屬問卷，敬請各位師長與同仁 務必於 1/20（含）前回覆問卷，感謝您的配合。相關說明如下：
問卷確認重點：(問卷內容主要針對以下事項進行統計，請依個人情況詳實填寫)
1.健檢參與意願確認。
2.是否參加「警覺型」或「高端健檢」（限50歲以上同仁）。
3.預計受檢校區：請於「中央校區」或「建國校區」中擇一填寫。(因涉及醫院人力配置，健檢當日請務必依照您所選定的校區前往受檢)

Greetings:
To safeguard the health of our faculty and staff, the annual health check-up for the 115 academic year is scheduled to be conducted between March and April. In order to ensure the medical capacity and quality of health check-up services at Tzu Chi Hospital, as well as to accurately allocate nursing staff across all campuses, the university must first compile statistics on the number of participants and their preferences for each type of health check-up.
This time, individual questionnaires will be sent via E-mail. We kindly request all faculty and staff members to complete and return the questionnaire by January 20th (inclusive). Your cooperation is greatly appreciated. The relevant details are as follows:
Key Points for Questionnaire Confirmation:
(The questionnaire primarily collects information on the following matters; please fill it out truthfully according to your personal circumstances.)
1. Confirmation of willingness to participate in the health check-up.
2. Whether you will participate in the "Alert" or "Premium" health check-up (limited to staff members aged 50 or above).
3. Preferred campus for the health check-up: Please select either "Central Campus" or "Jianguo Campus." (Due to hospital staffing arrangements, please ensure that you attend the health check-up at the campus you have selected.)
