一、依據衛生福利部國民健康署115年度補助地方辦理「整合性長者健康促進計畫-工作項目1長者健康促進服務」辦理。
二、計畫重點如下：
(一)申請單位：國內合法立案之社團法人、財團法人、公司、醫療機構、醫事機構、醫事團體、社福團體及公私立大專院校等。
(二)計畫目的：整合長者健康整合式功能照護(ICOPE)理念，發展全面性、具實證效益及地方特色之長者健康促進服務方案模組。
三、請依申請須知研提計畫，並於期限前將計畫提報表、計畫經費表、合作意向書及立案證明影本等資料各1式7份併同電子檔光碟1份提供給研發處智慧財產及產學創業中心。
四、本案相關附件表單可至本局網站公告區域或至雲端硬碟(https://reurl.cc/EbY1z0)下載使用。

**Announcement Regarding the Application for the "Integrated Elderly Health Promotion Program" Subsidy**
1. This program is implemented in accordance with the guidelines provided by the National Health Administration, Ministry of Health and Welfare, for the 2026 fiscal year subsidy to local governments for the "Integrated Elderly Health Promotion Program – Work Item 1: Elderly Health Promotion Services."
2. **Key Points of the Program:**
- **Eligible Applicants:** Legally registered social organizations, foundations, companies, medical institutions, healthcare organizations, medical associations, social welfare organizations, and public or private colleges and universities within Taiwan.
- **Program Objective:** To integrate the principles of Integrated Care for Older People (ICOPE) and develop comprehensive, evidence-based, and locally distinctive elderly health promotion service modules.
3. Please prepare your proposal in accordance with the application guidelines and submit the following documents in seven paper copies along with one electronic copy on a CD-ROM to the Intellectual Property and Industry-Academia Collaboration Center at the Office of Research and Development before the deadline:
- Project Proposal Form
- Project Budget Form
- Letter of Intent for Collaboration
- Certified Copy of Registration
