【導師請注意】1141學期學生操行評分
一、本學期操行評分將於114年12月31日(三)開放至115年1月7日(三)中午12點關閉。
二、請各位師長至校務資訊系統>學務處>生活輔導組>新增操行成績，相關作業介面請閱附件。
有設導師之班級>>請各位導師進行評分
未設導師之班級>>請系(所)主任進行評分
三、依本校學生操行成績考核辦法，操行成績計算評定公式如下：操行成績=基本分82分＋組導師共加減1~4分＋生活輔導組教官(校安)加減1~4分＋缺曠課及獎懲加扣分（導師評分學生表現，加減滿4分須於備註說明原因）。
四、感恩老師協助，有任何疑問，請洽學務處生活輔導組-分機11203。
備註：60分以下操行不及格者，預計於期末學生獎懲委員會提案。

**Notice to Faculty Advisors: Student Conduct Evaluation for Semester 1141**
Dear Faculty Members,
Please take note of the following regarding the student conduct evaluation for this semester:
1. **Evaluation Period**
The conduct evaluation system will be open from December 31, 2025 (Monday) to January 7, 2026 (Monday), closing at noon on January 7.
2. **Accessing the Evaluation System**
Please log in to the Administrative Information System > Student Affairs Office > Life Counseling Section > Add Conduct Score. For detailed instructions, please refer to the attached document.
- For classes with assigned advisors: Please complete the evaluation as your advisor.
- For classes without assigned advisors: Please complete the evaluation as the department (or graduate institute) head.
3. **Evaluation Formula**
According to the Student Conduct Evaluation Guidelines of this university, the conduct score is calculated as follows:
Conduct Score = Base Score of 82 points ± 1–4 points (added or deducted by class advisor) ± 1–4 points (added or deducted by Life Counseling Section instructor/security officer) ± points for absenteeism, tardiness, rewards, and disciplinary actions (advisors must provide a written explanation in the remarks if adding or deducting a total of 4 points).

**Note:** Students scoring below 60 points in conduct evaluation will be recommended for disciplinary action by the End-of-Term Rewards and Punishments Committee.
