一、 重要提醒：
1. 本項獎助金係依據《慈濟大學清寒助學金辦法》辦理。
2. 請同學務必提醒導師上網填寫導師意見表，做為審查依據。
二、申請期間：1141229-1150102止(學生資訊系統將於此期間開放申請，並須同步繳交紙本資料)
三、資料繳交地點與窗口：各校區學務處可代收。
※日間部：

四、申請資格：
1.學業（智育）成績須達 65 分（含）以上，德育（操行）成績須達 80 分（含）以上，且無小過（含三支申誡累計）以上處分紀錄（含銷過前紀錄）。
2.前一學期完成志工服務及人文時數合計至少 10 小時（不得重複計入申請本校其他獎助學金所要求之時數），並須於第 18 週（含）前完成登錄。
五、應繳資料：（繳交前請詳閱並確認資料正確）
1.學生系統申請書（含同意書）—學生本人務必簽名。
2.戶籍謄本正本(戶籍若不同請分別附上) 。
※114-2開學日為2月，戶籍謄本(需三個月內)正本需114年12月以後開立之，或新式戶口名簿完整影本（含父母/監護人與學生本人，含記事欄），若繳交戶口名簿影本，須另附內政部「戶口名簿請領紀錄證明」。
3. 三個月內開立之下列任一佐證資料（擇一提供，須含學生姓名）：
※清寒證明正本。(證明需為114年12月開立)
※低收入戶證明正本。(證明需為115年)
※中低收入戶證明正本。(證明需為115年)
4. 財政部國稅局核發之113年度所得證明（需含父母/監護人與學生本人）。

**Important Notice:**
1. This scholarship is administered in accordance with the "Tzu Chi University Merit Scholarship for Economically Disadvantaged Students Regulations."
2. Students are required to remind their academic advisors to submit their online advisory comments, which will serve as part of the review process.
**Application Period:**
From December 29, 2025 (114/12/29) to January 2, 2026 (115/01/02). The Student Information System will be open for applications during this period, and paper documents must be submitted simultaneously.
**Submission Location and Contact Window:**
Applications may be submitted at the Student Affairs Office of each campus.

**Eligibility Criteria:**
1. Academic (intellectual) performance must be at least 65 points, and moral (conduct) performance must be at least 80 points. No record of minor or more severe disciplinary actions (including warnings) is allowed (records prior to clearance are also considered).
2. A minimum of 10 hours of combined volunteer service and humanities activities must be completed in the previous semester (hours cannot overlap with those required for other scholarships). These hours must be logged by the end of Week 18.
**Required Documents:**
(Please review and confirm all documents before submission.)
1. Application Form from the Student System (including consent form)—must be signed by the student.
2. Original household registration certificate(s). If household registration differs, please submit separate certificates.
- For students whose semester begins in February (Semester 114-2), the household registration certificate must be issued after December 2024 (within three months). Alternatively, a complete copy of the new-style household register (including parents/guardians and the student, with notes section) may be submitted. If a household register copy is provided, an additional "Household Register Issuance Record Certificate" from the Ministry of the Interior must also be included.
3. Any one of the following supporting documents issued within three months (must include the student's name):
- Original certificate of economic hardship (issued in December 2024).
- Original certificate of low-income status (issued in 2025).
- Original certificate of medium-low-income status (issued in 2025).
4. Income certificate for the year 2024 issued by the National Taxation Bureau (must include parents/guardians and the student).
