送審證明同意書最慢於114/12/30下班前提供。
請計畫申請人使用最新版本(114.09.01)之申請表，

The deadline for submission of the "Animal Experiment Application Form" for the 115th academic year, as administered by the National Science and Technology Council (NSTC), is **noon on December 29, 2025**.
The consent form for submission review must be provided **no later than the end of business on December 30, 2025**.
Applicants are required to use the latest version of the application form (dated September 1, 2025).
