體育館使用證退費公告
因COVID-19 新冠狀病毒疫情影響，致使兩校區體育館4月份閉館，造成使用者之不便，
經校長同意後，本組將辦理4月份使用證退費。
相關退費申請流程如下，請各位使用者務必配合，如有任何問題，可再與課器組聯絡(8565301#1211)。
退費方式如下:
一、攜帶相關證明文件:
1.申請單或現場填寫(至校務系統下載填寫)
2.有效期間之使用證及申請單
3.職員證or學生證
4.身分證等可證明本人之證件
二、受理時間:05月11日至05月22日
平日09:00~11:30/13:30~21:00(假日不受理)
三、辦理地點:大喜館&人社院體育館櫃台
因費用結算關係，請各位申請人務必於期限內至指定地點辦理退費，
如逾時限，將無法辦理退費，造成不便 敬請見諒。

Refunding the Sports Center Fee
To mitigate the COVID-19 epidemic, starting on April 6, the University closed the Sports Centers on both of its campuses.
The University will refund you a portion of the fee, and the application process is listed below:
1. Application Process:
●Submit the application form to the Sports Center
●Present your Sports Center pass
●Present your ID card
2. Application period:
●May 11~22
●9:00 a.m. – 11:00 a.m. or 1:30 p.m. – 9:00 p.m., Monday through Friday
3. The refund amount is stated on the application from.
4. Any applications after May 22 will not be accepted.
