※送來之資料未齊全及缺件，則不予收件，請再送件前仔細檢查。

**Application Period:** Please submit the completed application form to the Student Affairs Office for official processing within three months of the incident. After obtaining the official seal, kindly mail the documents yourself. Late submissions will not be accepted.
**Eligibility Criteria:** Please refer to the attached document for eligibility requirements. Applications from cases of social dependency will not be accepted.
**Required Documents:** Please refer to the attached document for the list of required documents.
**Submission Guidelines:** Please provide only the documents specified in the application guidelines. Do not submit any additional proofs not required, in order to conserve paper usage and promote environmental protection, energy conservation, and carbon reduction.
**Important Notice:** If you do not meet the eligibility criteria mentioned above, please do not submit your application. All applications that do not meet the criteria will be rejected.
**Document Completeness:** Applications with incomplete or missing documents will not be accepted. Please carefully review all required documents before resubmitting. |
