一、依據「臺北市政府表揚運動有功團體及人員要點」辦理。
二、臺北市為表揚臺北市優秀運動選手、教練、推展運動有功之績優團體及人員，特訂定前開要點，並設有「傑出運動選手獎」、「優秀運動教練獎」、「績優體育團體及個人獎」、「運動推手獎」、「推動運動性別等獎」及 「終身成就獎」等獎項；本（115）年度訂於9月8日於臺北體育館舉行表揚典禮頒獎。
三、為循前開要點進行評選，請於本年6月30日前（郵戳為憑），免備文寄送推薦資料至臺北市政府體育局輔導管理科（臺北市松山區南京東路四段10號），並於信封標註「臺北市 115年運動有功表揚典禮推薦資料」，所送資料須含：
(一)紙本推薦表。（公告於臺北市政府體育局官網【http:／／sports.gov. taipei/】/公告資訊/最新消息，請下載列印）
(二)推薦表Word檔及相關照片、證明文件之電子檔，請以光碟片或隨身碟提供。
四、鑒於推薦資料初審作業已往多有接洽補件之需求，爰建請儘早投件，並可視需要，就上述推薦資料相關問題洽臺北市政府體育局馮先生（聯絡電話：02-2570-2330轉5422）先行 確認。
I. This announcement is issued in accordance with the "Guidelines for the Recognition of Outstanding Sports Teams and Individuals in Taipei City."
II. Taipei City has established the aforementioned guidelines to honor outstanding athletes, coaches, and distinguished sports teams and individuals who have made significant contributions to the promotion of sports. The awards include the "Outstanding Athlete Award," "Excellent Sports Coach Award," "Distinguished Sports Team and Individual Award," "Sports Advocate Award," "Promotion of Sports Gender Equality Award," and the "Lifetime Achievement Award." The award ceremony for the 115th academic year will be held on September 8 at the Taipei Gymnasium.
III. To proceed with the selection process in accordance with the aforementioned guidelines, please submit your recommendation materials by June 30 (postmark date will be considered). Please send the materials directly to the Guidance and Management Division of the Taipei City Department of Sports (No. 10, Section 4, Nanjing East Road, Songshan District, Taipei City), and clearly label the envelope as "Recommendation Materials for the Taipei City 115th Sports Recognition Ceremony." The submitted materials must include:
(1) A printed recommendation form. (The form is available for download and printing from the Taipei City Department of Sports official website: [http://sports.gov.taipei/](http://sports.gov.taipei/), under "Announcements" and "Latest News").
(2) A Word file of the recommendation form, along with electronic copies of related photographs and supporting documents. Please provide these on a CD or USB drive.
IV. Considering that there have been frequent requests for supplementary materials during the preliminary review of recommendation forms in the past, we kindly request that you submit your materials as early as possible.
