申請資格：務必須~已~申請115-1學雜費減免-低收入戶子女
※申請時間：115年9月7日起至115年9月18日止，繳交至中央校區學務處生
輔組林小姐，逾時不候。
※申請流程：慈濟大學首頁→學生資訊系統→學務處→生活輔導組→獎助學金
系統→申請（學產基金低收入戶助學金）→列印申請表暨同意書→繳至學生事
務處生活輔導組。
※檢附文件：
1.學生系統申請書暨同意書，請簽名。
2.教育部申請書。
3.三個月內申請之低收入戶證明正本（含學生名字）。臺北市請由臺北通
APP列印
4.前學期成績單。
5.獎懲紀錄（1150201-1150731）。

**Announcement: Application for the Low-Income Household Scholarship under the Academic Fund**
**Eligibility:**
Applicants must have already applied for tuition and miscellaneous fee reductions for the academic year 115-1 as low-income household students.
**Application Period:**
Applications must be submitted from September 7, 115, to September 18, 115, to Ms. Lin at the Student Affairs Office, Life Guidance Section, Central Campus. Late submissions will not be accepted.
**Application Procedure:**
1. Visit the Tzu Chi University homepage.
2. Access the Student Information System.
3. Navigate to the Student Affairs Office → Life Guidance Section → Scholarship System.
4. Apply for the "Low-Income Household Scholarship under the Academic Fund."
5. Print the application form and consent agreement.
6. Submit the printed documents to the Life Guidance Section of the Student Affairs Office.
**Required Documents:**
1. Application form and consent agreement from the student system, signed by the applicant.
2. Application form from the Ministry of Education.
3. Original low-income household certificate issued within the last three months (must include the student is name). For Taipei City applicants, please print the certificate via the Taipei Citizen APP.
4. Transcript from the previous semester.
5. Record of academic awards and disciplinary actions (from 1150201 to 1150731).
