一、 欲申請115學年度「大專校院弱勢助計畫-〔生活助學金〕」項目之同學
請於
1.申請資格:
(1).低收入戶。【檢附低收入戶證明】
(2).中低收入戶。【檢附中低收入戶證明】
(3).大專校院弱勢學生助學計畫通過審核之學生，以教育部每年公告查核結
果通過之名單。
2.申請期限
115年09月30日前完成以下程序：

(1).至慈濟大學學生資訊系統->總覽->學務處->獎助學金申請->
「大專校院弱勢助計畫-〔生活助學金〕」 填寫線上申請表
4.列印同意書並送「應附」以下文件
(1)線上與紙本申請表
(2)3個月內全戶戶籍謄本正本【須包含學生本人、學生父母或法定監護人；
學生已婚者，加計配偶】
(3)114年度綜合所得稅各類所得資料清單(可至各縣市國稅局或稽徵所申請)【
須包含學生本人、學生父母或法定監護人；學生已婚者，加計配偶】
(4)114學年度第2學期成績單【新生及本學期轉入之學生免計成績】
(5)學生家庭現況困難之相關證明文件。【若無家庭現況困難情形，則免附相
關證明文件】

Subject: Application for the "Assistance Program for Disadvantaged Students in Higher Education Institutions – [Living Allowance]" for Academic Year 115
Eligibility:
Applicants must have already applied for the "Tuition and Miscellaneous Fee Reduction" for Academic Year 115 as "Children of Low-Income Households" or "Children of Medium-Low-Income Households."
I. Students wishing to apply for the "Assistance Program for Disadvantaged Students in Higher Education Institutions – [Living Allowance]" for Academic Year 115 should complete the following procedures by September 30, 115:
1. Eligibility:
(1) Children of low-income households. [Attach proof of low-income household status]
(2) Children of medium-low-income households. [Attach proof of medium-low-income household status]
(3) Students who have passed the review for the "Assistance Program for Disadvantaged Students in Higher Education Institutions," as per the list published annually by the Ministry of Education.
2. Application Deadline:
Complete the following procedures by September 30, 115.
3. Application Method:
(1) Log in to the Tzu Chi University Student Information System -> Overview -> Student Affairs Office -> Scholarship and Financial Aid Application -> "Assistance Program for Disadvantaged Students in Higher Education Institutions – [Living Allowance]" and fill out the online application form.
4. Print the consent form and submit the following required documents:
(1) Online and paper application forms
(2) Original household registration copy within the last three months [must include the student, the student's parents or legal guardian; if the student is married, include the spouse as well]
(3) 114 Annual Comprehensive Income Tax Data List (available from local tax bureaus or tax offices) [must include the student, the student's parents or legal guardian; if the student is married, include the spouse as well]
(4) Transcript for the second semester of Academic Year 114 [new students and those who transferred this semester are exempt from submitting transcripts]
(5) Relevant proof documents demonstrating the current financial hardship of the student's family. [If there is no current financial hardship, no proof documents are required]
